Your WindoPath LIS Isn't Losing You Money. Your Billing Handoff Is.

No pathology group bought WindoPath expecting it to argue with payers. That was never the job; it accessions the case, tracks blocks and slides, stores the diagnosis, and delivers the report. The money slips away after sign-out, in the gap between what the pathologist examined and what the claim reports. Most practice managers don’t catch it until a denial batch lands or aging passes 90 days, and by then it isn’t a fix, it’s a write-off.

Here's where it actually happens, by platform:

TransDontics Table Preview
Where It Happens
What Happens
Real Cost
Case Accessioning — Specimen Parts & CPT Level Drift
Each separately identified specimen earns its own 88305 unit, but several fragments in one container count as one. Parts merged or split wrong at accessioning change the bill. Prostate cores need G0416 for Medicare and 88305 per core for many commercial plans.
Specimen-unit errors are the largest recoverable category we find in first pathology audits.
Stains, IHC & Frozen Sections — Units the Report Doesn't Show
IHC bills 88342 for the first antibody per specimen, 88341 for each additional one, and 88344 for multiplex. Special stains split between 88312 and 88313, and frozen sections need 88331 plus 88332 per extra block. Levels and recuts aren't billable, yet they still slip onto claims.
Groups that bill stains from memory leave money behind and invite refund requests. Clean does not always mean correctly paid.
Shared AP & Clinical Database — Tox, Micro & e.outreach Orders
WindoPath keeps AP and clinical work in one database, so one patient can carry pathology, toxicology, and microbiology charges at once. Orders from e.outreach often arrive without insurance or a usable ICD-10 code, and definitive drug tests still need the right G0480 to G0483 tier.
This produces the denials nobody can explain. The case was billed as always; the payer just split the services differently.
Every Lab — Global vs. Split Billing, Direct-Bill Laws & Timely Filing
Pathology bills global, technical (TC), or professional (26) depending on the client, and several states require anatomic pathology to be billed directly. Miss these rules and the claim goes out clean and comes back denied, while lag eats into the filing window.
Timely filing denials are avoidable almost 100% of the time. They're a process gap, not a payer decision.
None of this means WindoPath is doing anything wrong. It means pathology billing is its own discipline, built around how WindoPath actually records parts, blocks, and stains, and that’s what most AP labs are missing.

Not sure which of these is actually happening in your claims?

That’s exactly what the free audit below is built to find out.

Frequently Asked Questions

What is WindoPath, and what do WindoPath Ē.ssential, its specialty modules, and e.outreach each do?

WindoPath is the anatomic pathology LIS from Psychē Systems, a Hopkinton, Massachusetts company selling AP software since 1983. WindoPath Ē.ssential keeps AP and clinical work in one database, with modules for cytology, histology, dermatopathology, GI, urology, toxicology, and microbiology. Extension modules add digital pathology, QC, and document scanning. e.outreach handles ordering and results across locations, and WindoPath runs locally, Psychē-hosted, or in the cloud.

Do we need a specialized billing company if our lab already uses WindoPath?

WindoPath manages cases, slides, and reports, but it doesn't guarantee a clean, correctly paid claim. Most pathology billing errors happen in the handoff between what was examined and what gets coded, which is why groups on WindoPath benefit from WindoPath billing services run by people who know those patterns.

Can TransLabs Billing work with our existing WindoPath setup without changing our workflow?

Yes. Our onboarding maps to your existing case types, specialty modules, billing interface, and client agreements rather than asking your lab to change how it already runs. Your histotechs and pathologists keep working exactly as they do now.

What does the free WindoPath billing audit include?

A review of sample cases, part counts, stain billing, and coding data from your WindoPath environment to find denial trends, missed units, and coding gaps, delivered at no cost and with no obligation. You get the findings in writing.

Do you handle dermatopathology, GI, cytology, and toxicology billing on WindoPath?

Yes. We manage per-specimen 88305 units, IHC and frozen section coding, prostate biopsy billing by payer, cytology screening versus diagnostic codes, drug test tiers, and TC and 26 splits, alongside standard commercial, Medicare, and Medicaid claims.

How much does it cost to outsource pathology billing to a company like TransLabs?

Outsourced pathology billing is usually priced as a percentage of collections rather than a flat fee, which keeps the incentive aligned; we only get paid when you get paid. The exact percentage depends on case volume, specialty mix, and how much A/R cleanup is needed. We quote a specific number after the free audit, since a rate quoted without real claims data isn't worth trusting.
