Your AP EASY System Isn't Losing You Money. Your Billing Handoff Is.

No pathology group signed up for AP EASY expecting it to argue with payers. That isn’t its job; it accessions the case, labels the cassettes, holds the diagnosis, and sends the final report. The money leaks out after sign-out, in the gap between what the pathologist actually examined and what the claim says was done. Most practice managers miss it until denials land or aging passes 90 days, and by then it isn’t a fix, it’s a write-off.

Here's where it actually happens, by platform:

TransDontics Table Preview
Where It Happens
What Happens
Real Cost
Case Accessioning — Specimen Count & CPT Level Drift
Every separately submitted specimen earns its own 88305 unit, so one merged part at accessioning costs a full charge. Case types left on old CPT levels after January bill wrong, and prostate biopsies need G0416 for Medicare but 88305 per core for many commercial plans.
Specimen-unit errors are the largest recoverable category we find in first pathology audits.
Stains & IHC — Per-Specimen, Per-Antibody Units
Immunohistochemistry bills 88342 for the first antibody on each specimen and 88341 for each additional one, with 88344 for multiplex stains. Special stains split between 88312 and 88313. Stains added after sign-out often never reach billing.
Labs that bill every stain the same way leave money on claims that technically pass. Clean does not always mean correctly paid.
GYN Cytology & Molecular Reflex — Screening vs. Diagnostic Codes
AP EASY can reflex a Pap to HPV testing automatically, but billing still has to sort screening from diagnostic. Medicare screening Paps use G-codes, diagnostic Paps use 88175 or 88142, and the HPV reflex on 87624 or 87625 needs a diagnosis that supports it.
This produces the denials nobody can explain. The Pap was billed as always; the screening rules or reflex criteria just didn't match.
Every AP Lab — Global vs. Split Billing, Direct-Bill Laws & Timely Filing
Pathology often bills global, technical (TC), or professional (26) depending on the client. Several states require anatomic pathology to be billed directly, not through the ordering office. Miss these rules and the claim goes out clean and comes back denied, while lag eats into the filing window.
Timely filing denials are avoidable almost 100% of the time. They're a process gap, not a payer decision.
None of this means AP EASY is doing anything wrong. It means pathology billing is its own discipline, built around how AP EASY actually records cases, specimens, and stains, and that’s what most pathology labs are missing.

Not sure which of these is actually happening in your claims?

That’s exactly what the free audit below is built to find out.

Specimen count reconciliation, CPT level mapping by case type, G0416 versus per-core prostate billing, and daily matching of signed-out cases to claims.

IHC, Special Stain & Molecular Reflex Coding

Per-specimen and per-antibody IHC units, special stain groups, add-on stain capture, and HPV reflex coding with the diagnosis to support it.

Specialty AP Billing

Frequently Asked Questions

What is CGM AP EASY, and which pathology specialties does it support?

CGM AP EASY is a cloud-based anatomic pathology LIS from CompuGroup Medical, which acquired AP Easy in 2022. It runs on Amazon Web Services in any browser and supports surgical pathology, dermatopathology, GI, urology with a prostate module, and GYN and non-GYN cytology. It connects to EMR and billing systems through HL7 or API links.

Do we need a specialized billing company if our lab already uses AP EASY?

AP EASY manages cases, reports, and data exchange, but it doesn't guarantee a clean, correctly paid claim. Most pathology billing errors happen in the handoff between what was examined and what gets coded, which is why groups on AP EASY benefit from AP EASY billing services run by people who know those patterns.

Can TransLabs Billing work with our existing AP EASY setup without changing our workflow?

Yes. Our onboarding maps to your existing case types, report templates, billing interface, and client agreements rather than asking your lab to change how it already runs.

What does the free AP EASY billing audit include?

A review of sample cases, specimen counts, stain billing, and coding data from your AP EASY environment to find denial trends, missed units, and coding gaps, delivered at no cost and with no obligation.

Do you handle dermatopathology, GI, and urology pathology billing on AP EASY?

Yes. We manage per-specimen 88305 units, IHC and special stain coding, prostate biopsy billing by payer, cytology screening versus diagnostic codes, and global, TC, and 26 splits, alongside standard commercial, Medicare, and Medicaid claims.

How much does it cost to outsource pathology billing to a company like TransLabs?

Outsourced pathology billing is usually priced as a percentage of collections rather than a flat fee, which keeps the incentive aligned; we only get paid when you get paid. The exact percentage depends on case volume, specialty mix, and how much A/R cleanup is needed. We quote a specific number after the free audit, since a rate quoted without real claims data isn't worth trusting.
